This is the whole binder in one sentence: minutes are a record, not a summary. The most expensive thing AI does to a transcript is promote a discussion into a decision, because summarizing rewards tidiness and the record punishes it. So write the bans. Nobody "decided" unless somebody said so. Two people agreeing is two people agreeing, name them. "We should probably look at that" is not an action item, it's a sentence somebody said. Split the page into DISCUSSED, DECIDED, and NOT RESOLVED, and make it justify everything under DECIDED.
